There is the expected conflation of sex and gender, though some of the questions are open response which is useful.

So much assumption that gender is a thing!

So many of the questions are about 'LGBTQ+' so it's really difficult to answer when my views on 'LGB' are so different from my views on 'TQ+'.

Agree that it's hard to answer some of the questions from a GC POV.

It's a totally shit survey, since it conflates homophobia with transphobia, but I made ample use of the write-in boxes.

It's a totally shit survey because the qualifying questions don't give me an option to say that I wasn't assigned a gender identity at birth, my sex was observed.

TheHoliday that's my issue also. My birth certificate, my passport, my driver's lincense, my foreign permit card all list my SEX. No gender anywhere and my sex most certainly wasn't assigned it was observed and noted.
