It's a bit old (originally published in 1979 when my younger siblings had a copy then updated in the 90s) but Claire Rayner's The Body Book covers it among other things such as poo-ing, wee-ing, breathing, emotions, senses, getting older and dying. It's still available on Amazon www.amazon.co.uk/Body-Book-Non-fiction-Claire-Rayner/dp/0590556088/ref=sr_1_fkmr1_1?ie=UTF8&qid=1499011535&sr=8-1-fkmr1&keywords=clare+rayner+the+body+book&tag=mumsnetforum-21
Sex and puberty are in the context of where babies come from, and this is only one part of the book. I remember my brothers and my own children being more interested in the chapter on the digestive system than the other chapters.
It does refer to the vagina as the baby making hole, which may piss you off.

I had this book as a kid. It was better than others I came across.

I got this book from the library and was amazed by how good it was so i bought it. I dont think there's anything in it that has pissed me off and I'm very fussy about this sort of thing!

There are lots of other books in the series as well. All the ones I've seen are great.
In case the link doesn't work, it's called It's Not The Stork and it's by Harris, Robie, H.
I got it when my DD was 6 and it was perfect.

We've got 'Where Willy Went' which I love! A nice gentle introduction!

Amazing you - getting to know your private parts, which I think is excellent, very factual, but not too much!

And lastly, What's the big secret? Talking about sex with boys and girls, which again is very good.

The link i posted was for amazon.co.uk because i'm presuming you're in the uk but If you look it up on amazon.com you will be able to do a "look inside the book." and then if you like it you can buy it from amazon.co.uk (if you're in the uk).
I like the way it has a "girls to this, boys do that" page where it explains that girls and boys can have long or short hair, play with trucks and dolls etc etc and then goes on to explain the real i.e. biological differences between boys and girls.
Here is the link for amazon.com where you can look inside the book....

Usborne and American Girl (don't be put off by the name) have some good ones.

We just had a big, orange hardback book (also by Claire Rayner) that just generally covered the human body. It had really beautiful black and white pencil drawings for everything, and I remember mum showing me how babies grow in it (I can still picture the little squeezed up face of the baby in picture of birth)

Sometimes I think that factual text books can be just as good as stuff aimed specifically at kids.

Ha! Found it! (I may have to buy it just for the memories)
