Hiya,
is it possible to transfer my old data from the nhs app to a new phone? I’ve got a very lovely looking track history which I’d like to keep tracking.

If you do a backup of your old phone and then upload it to your new one, all the apps and data should be there

You can install the app on your new phone and access everything as it is. When you get a new phone it will have software to transfer everything to your new phone if not just go on your app store and download it.
