Ballooning over lockdown...help needed

I've always been fairly big, but felt "fit" with it. However, this last year I've ballooned to my biggest ever weight. I'm so ashamed, I've hit over 19st. I feel terrible healthwise and have lost any fitness that I had. After a year WFH and living in leggings I can't fit into any clothes either.

I lost my dog at the start of the year & have struggled to motivate myself to do an hours daily walk without him. I've always been sociable, but I don't want to be seen out and I'm dreading social events now they've restarted.

I've been on and off diets since I was 12, never found one that I can stick to and with PCOS find it so difficult to shift weight. Keto or VLCDs are the only ones that I've ever had any success with, but struggle to stick to them for more than a month or two.

Any help, motivation or just a kick up the backside gratefully received. I'm disciplined in all other areas of live, so I don't understand why I have such a block when it come to my own health?
