You get a point for each stone you weigh don't include the lb (10st 6lb = 10 points)
Your height
Under 5ft 4ins = 1
5ft 4ins and above = 2
In day to day life do you
Sit down most of the day = 0
Occasionally sitting but mostly standing = 2
Walking most of the time = 4
Doing physically hard work most of the time = 6
Do you want to
lose weight = 0
maintain your current weight = 6
