My passport has expired ... im going from my married name back to maiden name... what do I need to send off??

Go through the online renewal process - one of the questions is “are you changing your name / details” or something like that. Say yes and follow it along from there. You’ll be told what you need

It’ll tell you on your application. Are you using deed poll or your divorce decree as proof for all other companies? Send the same proof.
