Health Questionnaire

Our Head's new PA has emailed me to remind me to complete a Health Questionnaire. I didn't fill it in because I thought it was intrusive or a joke. In my 22+ years at the school, I have never been asked to do this before and I feel quite angry about it. The last time I had time off for illness was around 6 years ago and it was a 24 hour D&V bug so it's not as though I'm a piss-taker.

Do I have to do it? There is no option for "refused to divulge". I don't see what right she has to know this personal information. TIA

This is outrageous. What a nerve.
No way I'd be completing that. I'd continue to ignore and if she pushes it then start by asking what the purpose of the data collection is, what will the information be used for, how will it be stored, who will have access and when it will be destroyed.
Has everyone else completed it?

Not sure @HeyJackKerouac if anyone else has completed it. I do have a reputation for being outspoken, but as a 50+ female originally from northern England, I speak as I find and if the soft southerners round here don't like it, it's a them problem as my DDs say. I just wondered if I was being awkward for the sake of it, so thanks for being suitably annoyed too.

That's outrageous! I think a conversation will need to be had. I get on pretty well with the Business Manager and his name is on the form so I will try to speak to him when I am in tomorrow. Glad it's not just me being stroppy!
