Sick note information on return to work?

I was off work on Monday and Tuesday this week with a virus, I don't work on Wednesdays. I will be returning to work tomorrow (Thursday) and will be required to fill in a form to give to personnel.

Do I just write the two days Monday and Tuesday on the sick note, or do I have to also add the Wednesday as a third day (because the date of my return will be Thursday)?

Sorry for asking a strange question, I just want to be sure of my facts before I return to work.

You mean a return to work document?

Yes - as Wednesday is not a working day for you.

If it asks for a return date though then it would be the 25.01.2024…….

Yes, the form you sign when you return to work to say why you were sick and the dates.

I know the return date will be 25.1.22, but I am wondering if that means they will put me down for 3 days of sickness instead of 2 (as today is not a working day for me so therefore I am not taking it as a sick day).

Thank you for replying, and sorry if it is obvious. I just don't want to have more sick days put on my record than I actually took.

last year I was extremely unwell and off for almost the entire term.

I was due to go back after the half term holiday, but I needed an operation which I had during half term. I was only signed off until my last working day of the half term (im also PT)

Then i had my operation at the end of the half term, then had another 6 weeks off after.

My first sickness period ended on my last working day of the first half term.
The you can see I was "well" for a week or so (during the half term holiday) and then I had a second period of sickness after my operation.

It was 2 periods of sickness but I never actually returned to school in between, and just did 2 forms when I finally went back to school.

My form shows me as being "well" in the middle, and I didn't have any extra days recorded off sick that the Dr had not signed me off for, and my first day back at school was a few days after my sick note ran out due to my work pattern
