Honesty job applications after capability

I was placed on informal capability in November, attended a capability hearing in Jan. There were some a bit funny goings on and I don't want this to be outing but ultimately I was offered a settlement which I accepted with an agreed reference.

I'm now looking to apply for jobs but what do I put in the "reason for looking for other job" box and do I say I have "been dismissed or forced to resign"?

To me the honest answers feel like "other school doesnt want me" and "yes", but I'm not sure with a settlement that is technically/legally the case.

I'm naturally inclined to be honest but also I need to be able to get another job. I'd also have to tick to say all the information on the form is correct and if it turns out not to be I could be dismissed from the job which is worrying me.

Any advice on whether it's best to be upfront about these things on the application form / an interview or if that will just lose me any chance I had of getting the job would be appreciated.

Reference forms usually ask whether a candidate has been subject to capability.
You're looking for a new post because of a change in personal circumstances.
