I'm not a teacher but I need your views please.

My dc has had a couple of issues this week at school.
I feel they need to be followed up, but am aware it's very much one person's word against another.
She has asked me not to do anything as she fears repucussions and she has had such a difficult time settling in school (year 9) she doesn't want to rock the boat.
What happens if a parent doesn't complain as such, but brings a matter to the attention of the head?

The head isn't the right person to complain to.

Re the chair and toilet access, email your DD's head of year and ask for a reminder to be sent to staff about your DD's medical arrangements. Then if the teacher goes against them again you have a clear written trail to follow-up on.

About the pens, I'd just let it go. And the homework. The teacher hasn't lost the homework, she just hasn't got around to marking it and it's not a priority. Teachers are usually snowed under with marking, especially
English. The year's nearly over and unless your DD has her next year and she 'loses' some GCSE work, it's not worth the hassle.
