burn out ( have been off ) - now scared to go back because...

I work in secondary school. My school said that I underperforming and that they may start capability. they said i look tired and that i might be ill. I checked iwth my GP and he signed me of for exhaustion for 1 week. I do feel the burn out will take time to recover but i do not want to take more time off ( I was going to interviews/ took a lot of time off/ my school was not happy and now taking more time off because of burn out is probably risky). I thought 1. to book appointment with the line mmanager and say I am still not 100% but with school's support I would like to go back to work or 2. ask GP to say to issue a note that I work on a reduced hours?

Speak to your union ASAP. I would suggest that unless you want to leave on a comparise agreement you go back to your GP and get signed off with stress. I suspect you are more ill than you think.
