5mth pup peeing in bed overnight

We have a 5mth old Golden Retriever, she's been crate trained from the beginning and was doing really well but she got a urine infection when she was about 3.5 months old and therefore lost her control. Although this has cleared up long ago we're still coming down to a wet bed in the morning... it seems like she's lost the instinct to not pee in her bed.

Does anyone have any idea how to break the cycle again?

Have the crate in the bedroom with you so that when she stirs in the night you hear her and can whip her outside.

I would go back to the beginning and get up in the night to take her out like you used to when she was tiny.
