My two-year old Cav is pretty reliably toilet-trained, but if we don't actively encourage her to go outside and stand with her, she doesn't do anything that makes it obvious she wants to.
Last night I took her out for five minutes not long before I went to bed, she bimbled about for five minutes and (I assumed) peed. Half an hour later, shattered, I got off the couch to go to bed. She made her way towards the back door, as I usually put her out JUST before I go up. I assumed she didn't need a wee as she'd not long gone out, so ignored her and she just followed me up.
Then she surreptitiously peed on my bed 
I'm decorating ATM and have a new carpet and bed delivered today, which I'm keen to keep clean, obviously.
Was it my fault for not taking her out AGAIN just before bed? Do you think she realised I was not quite ready to go up the first time so didn't think that was her "pee window", IYSWIM?
Is there any way to train her to ASK rather than wait for me to anticipate and do her thinking for her?
I'd be grateful for any advice
