Any French speaking dog people about?!

We're taking our dog to France for the first time next week. Have located a vet for the return journey health check but need to know what to ask for when I ring to book the appointment. My pronunciation is okay I just need to know what to say!
Can anyone write me a sentence asking for an appointment with the vet for a pre travel health check and tapeworm meds.

my french is pretty basic but I'd suggest

Je voudrais un rendez-vous avec le veterinaire pour (date). Je vais voyager (date) à (Angleterre?) et je dois avoir un examen et les medicament pour les ténia pour mon chien.
