Self inflicted black eye

There is a long back story to this, but I'm just trying to figure out if anyone has a teenage son who has given themselves a black eye?

This has happened 3 times in the last 4 years that I'm aware of. They are definitely self inflicted but I just can't get to the bottom of why he does this to himself. I've tried various support, all which have been pushed away.

I think the back story would be pretty important in getting to the bottom of it, surely it doesn’t really matter what experiences other random people have had?

What various supports have you tried? Does the child claim to you that someone else did it, do they claim to friends that someone at home did it? Have you told them you know they did it themselves - how do you know, how do they react?

Are they trying to look cool or hard, or do they want sympathy/pity? Do they display it proudly, or try to hide it? Do they self harm in other ways?
