Any NHS primary care admin experts here? Teenager self GP registration - duplicate NHS number?

I'm trying to work out what's going on with my now 17yo - who I think was 16 at the time this happened - it seems she signed herself up at a different GP surgery (her girlfriend's) and was allocated a new NHS number when she already had one and was registered elsewhere in the same CCG - how could this have happened? I am going to ask her about it but timing is bad for a difficult convo and she's being quite evasive, so i'm just trying to work out in what scenario this could happen, and whether the surgery could have messed up or whether she must have deliberately misled them in some way.

I'm also spiralling a bit as to what could be so secret she didn't tell/ask us and possibly sought an emergency appointment about, it's also caused knock on family admin effects which I've been trying to deal with and the info about this GP registration I only found out this week, from her Dad, which he only found out because of a certificates for travel abroad situation he's having to try to sort out. Any insight appreciated!

It’s pretty unusual. Normally when
you register, the new practice search for you on the spine and make sure it’s a match before they pick your name.

New NHS numbers are really only issued if people have never had one before. (Sometimes for new identity reasons).

I would guess the most likely scenario is that they searched and found someone with the same name and dob as your DD and didn’t check carefully enough. They should check previous address to verify.

Either that or she gave them a different name and told them she’s never been registered with a GP.

Its going to be tricky to sort out if she won’t cooperate as they probably won’t talk to you due to her age.
