Concerned about Dd

My 14 year old dd's hair appears to be falling out at an alarming rate recently, probably since the beginning of this academic year. She is a very intelligent girl, but also very ambitious and a perfectionist and since the start of year 10 she has seemed slightly less like her normal, happy self. I have found a food diary in her room, in which she lists what she eats in a day and her total daily intake, which varies between 400 calories and 1,200 calories.This is obviously quite low, considering she is 5 ft 10 and moderately active. The reason I mention her hair loss is because I have read that it can be a result of an eating disorder, and this combined with the food diary worries me, although I haven't noticced any obvious weight loss. What steps should I take from here? I don't want to upset her further, but I think that she needs help, what is the best way to go about this?

Go and speak to your GP as soon as possible.
