Yes in Guildford - www.surreygp.com (on phone and can't seem to do links, sorry)

Dr Lewis is very good, takes time to go over all the pros and cons with you and gives you a small amount of Botox to start with and you go back in for a top up a couple of weeks later.

It's not the cheapest place - I paid £280 for 2 areas but she's a proper Dr.
