Can anyone recommend a REALLY good hand cream please?

I've tried loads and my hands are still dry. I don't mind spending up to £20 for something that will soothe my poor sore hands!

I have to apply hand cream many times per day because of medical condition.
Aveeno is best of the cheaper range.
Crabtree & Evelyn Gardeners hand cream excellent though costs £20+
I also use Udderly Cream followed by a layer of Vaseline.
