Help- open pores?

I have lots, they seem to get worse each year

Really grateful for:

1. Advice on how to shrink them (is this possible?)
2. Makeup thats good at concealing them

I've started using this from REN and my pores look smaller. I use Bare Minerals foundation with a Laura Mercier primer underneath and that helps a lot.
