My son is 16 and we are in the middle of getting his 1st passport but my question is we changed his name by deed poll to his step dads name but his birth certificate is blank and the passport form do i leave the father bit blank or put his step dads details in.

I don't think you can put his step dads name in because he isn't legally his dad is he

I'd leave it blank if there isn't anything on the birth certificate

Ok thank you i will just send the deed poll with his name change and leave the other bit blank.

Is there still a bit on the form where you can explain circumstances (eg why your leaving the blank space)?
When I filled dc passport in about 7 years ago I put an explanation in on the advice of the passport office. Mine was different it was due to doner insemination.

If DH hasn't adopted him and doesn't have parental responsibility then I would leave it blank.

I applied for my sons first passport and he has a deed of name change document too. I just had to send evidence of my child using that name. For example , school report, doctors letter etc etc

Ahh thats good to know i can send his exam letter as thats got his new name on it..
