Stepmother+biological father+ worries

Can Stepmother and biological father go to family therapy with small child without biological mother’s consent (won’t be given despite significant worries about the child).

No. Both dps need to consent to therapy sessions ime.

Not without a court order I think, I’m sure both parents (with parental responsibility) would need to consent.

Does one parent act as primary carer or is it 50/50?

5.2 Issues of informed consent, and who has the right to give it, are complex
in the treatment of whole families. Age is only one factor in whether an individual has the right to give or withhold consent on their own behalf or that of others. Therapists must consider all relevant factors carefully and be able to justify their actions.

Sounds like it’s worth contacting some therapists and explaining the situation to see if they’d be able to help.
