I've volunteered to help a parent through the statement process BUT I realise now how little i know or remember (maybe i've blocked out that statement trauma).

Anyway they have received the "parents views" form, this is the intial one sent out to parents by the LEA following request for SA by the school.

Her DC has a DX of ASD (moderate i'm guessing).
Whata re the essentials that she must include on this form.
How much detail should she include?
Is there any dx specific info that she could use (my ds has other dx so had other stuff to include)
Are there any good on-line example formats?

Thanks for your help, now I've said i'll hold her hand I want to make sure i'm offering the best support

A summary and chronology of the History from Birth to Present day. Details of Diagnosis and Difficulties backed up by professional reports and IEP's. A rough explanation of What Help/Support is needed and Why. How child is at home and how difficulties that Impact on his Education at school effect Home Life. Brief Sibling info and any difficulties listed that they may have. Professional contacts that are seeing the child and a sentence giving permission to contact.
Don't have to do it precisely by form just be roughly guided by it and write your own report. Then you have enough room to write what as much as you need.

Thanks Wasuup, I can remember following a bullet point heading type list for ds2 but can't for the life of me find my copy of his form... or maybe that was in my naive days before I copied everything.

Just done one and I wrote about 12 pages. On the form the most important part ,which is what support will be needed at school, is right at the end so don't run out of steam before then.

Not yet qualified to advise as still in process: My LA has agreed to begin process but are saying he might get a statement or he might remain on School action plus.

Very kind of you to help out. I got the nursery, his childminder and private ot to read through all or parts of it before sending. It was good to get different angles.

Your welcome actually doing my sons at the moment after the LEA refused to assess at first but backed down 9 days before it went to Tribunal.

Ive done the headings
History -sub headings Early Years, At School, At Home
Difficulties-diagnosis, quotes from reports, meetings and IEPs
Help/Support needed for his SEN
Professional Contacts-permission to contact
Professional Reports
Tis about 11 pages- need to add a little more and do some editing as well so maybe will be around 12 pages when completed.
