requesting an assessment for statement:

on yellow paper, question #2:

  1. what special educational needs does DS5 have?

(surely it doesnt mean Dx does it?)
(or is it breif overview of his difficulties?)

sorry ive no idea as you know but id fill the gap with everything you can in my opinion, though should add this is not based on any knowledge!!

Give brief overview of his difficulties and make it on his worst possible day. If you have no diagnosis but are in the process/on waiting list, say that. If you've had any relevant info from paediatrician or docs, say that.

On ours we put about DS's dx plus all the problems this causes him in a learning environment - all the things he needed help with rather than being left to try and cope alone. It maybe that your child needs actual physical help to go to the loo for example or climbing stairs in school or that they require support in learning, like extra reading practice or help in processing instructions. HTH WMxx

Also, like with DLA form, put what would happen if his needs were not met - to him and to other kids/ staff.
