DLA form - help needed....

So - having spent most of the weekend completing the form, have come to section 36 [!!!]

Should I be getting an statement from someone? will it make a difference? if so who should I ask?

Thanking you lots in advance for any words of wisdom you might have!

My statement was done by a good friend who has known DD all her life and happens to work at her preschool. It can be anyone who knows your child.

We got it awarded without the statement - DS's epilepsy nurse went on holiday and was going to send it when she got back, but we had been awarded by then! I was utterly amazed as I thought that without we wouldn't get it. Guess it depends on how many other professionals they contact perhaps?

I put my DS's SENCO. I don't think they contacted her though and we were awarded it.

My son occupational therpaist did his but i've only sent mine off , but it doesnt have to be filled in , good luck
