I am filling in the form on line.

Children don't have NI numbers do they?

Form is asking for an NI number for the person I am caring for. Should I just put the reference number that is on the DLA award letter?

Also for his title do I put Mr, he is 6 years old!

When they ask from which date you started caring for them do you say their birthdate or the date they were diagnosed? Thanks for any responses.

The reference number on the DLA letter is your child's NI number. Should be 2 letters 6 numbers and then another number.

Mr is fine for a little boy, and put their date of birth for whe you started caring for them.
