Ds's Statement -Parent Report

I am writing the parent report for DS's statement. I've written I have got a bit stuck - I want to make it as detailed as possible but I'm stuck on what to write. I cant find any sort of guidance only a very brief one provided in the LEA letter.

Durham gave me quite a long list, outlined as best as my 2 year old will let me:

Early years, when we realised there was a problem.
The help we've sought and had
The child's routines and relationships within the home and outside home.
Child's play abilities.
Social and communication abilities/difficulties
Behavioural difficulties and how we deal with them best
Emotional state - dealing with emotions, any signs of depression, anger, detachment, etc. Obsessions, compulsions...
Your feelings about progress and concerns for the future. How and where you would like your child to be educated and why.
Opinion the child, if old enough.
