Blue badge application form help please!

My brain is stuffed with cotton wool today and I can't think straight. Ds1 gets higher rate mobility so is automatically entitled to a blue badge.
I got the form today and this confirms this. So does anyone know if I need to fill in part C then that asks all the questions about how far he can walk, etc?

No, I dont think its nec if you get HR DLA-I didnt fill it in and still got the blue badge.

thanks, will jsut do that then. Realyl didnh't fancy another assessment of what he can or can't do!

I only filled in the first page on mine, then ticked the qualification box. Enclosed two photos, DS' entitlement letter and a cheque for £2 and Bob is your mother's brother.

Yes, riven - there was a box in part b. Just wasn't clear whether or not to leave part c. I'm just going to leave it.

Good idea about the cheque too. Ours says they will ask if the app is sucessful but I had might as well send it.
