Typical care plans for all the places I work include the following:

Mobility needs.
Toileting and personal care requirements.
Feeding neeeds.
Medication.
Communication.
Behaviour problems and how to handle them.
Cultural and religious needs.
Moving and handling risk assesment.
General risk assesments.

I don't think I have left anything out and hope this helps.
