Info and model letter on the site I linked to. The website has loads of information about SEN, schooling and statementing.

All Councils have an organisation called Parent Partnership that can give advice about school and kids with special needs, as they aren't funded independently then you have to bear in mind they aren't necessary impartial, but they may be useful to advise you about local procedures etc.

You might also want to think about (and talk with ed psych about) whether you want him to start a mainstream school or go to a special school or mainstream school with a unit for kids with special needs.
