help with dla form?

Getting through the form but am really stuck knowing what they mean in one of the sections. DD has autism (not specified where on spectrum yet but prob Aspergers or HFA) and poss ADHD and Oppositional Defiance Disorder. The section relates to help the child needs when they go out during the day or in the evening. Theres a separate box for three different things. Whaqt things should go in the boxes? is it particular activities ie if she egoes to tumbletots etc or school or just general ie play etc? Any help please its the last section apart from the big one at the end that i have to complete.

A tip - I do mine on the computer in Word, with the same section headings etc, and just send that along with the actual form with a 'see attached' note on it. All I put on the form is the basic information. If it's done in Word you can go back and edit as many times as you need until you feel you've got it right, plus you've got a copy to hand for when you renew.

i put things like going to softpay cos dd would hit other children, not going to parties for loud noise cos she would meltdown, not going to other ppl houses without me cos of her lack of sense of danger etc...
