EHCNA request form - help please!

My nursery SENco put my DD forward for the SEN under 5’s meeting and the outcome was to gather more information about her. They’ve sent me a form for a EHCNA request, it’s all very detailed about DD. I’ve got to fill out a section about her. What I’d like them to know. So I write the good and the bad? The meltdowns , sleep issues etc?

Think about strengths/needs, what DD likes/dislikes/what makes her happy, what is important to DD/you and what is important for DD, what is working well and what is not working so well/hasn’t worked in the past, how DD communicates/how others communicate with her, what she wants to do in the future/aspirations, any important history. Depending on the form, a bit about family/any other important people in DD’s life.

You don’t have to use the LA’s form to request an EHCNA. You can use IPSEA’s model letter if you want.
