Over-granulation....

My daughter is not responding to the steroid cream that we have been given to treat her over-granulation at her g-tube site.

So I was wondering what creams other people were prescribed?

You can use silver nitrate but it can be sore and may irritate especially around open wounds like stomas and G-tube sites. You can get it in pencil form, and you moisten the tip and paint it on the overgranulated area.

The other option is to see the community nurse/practice nurse and ask for some Lyofoam - this is a dressing that is pretty good at reducing overgranulation. You can cut it and put it around the site.

The problem is with continued use of lyofoam and other such products the child can become allergic to them - this has happened in our case.

As for the silver nitrate, there is only a number of times the treatment can be applied, and weve had our quota....

No I was just hoping that someone might be using a different steriod cream to try and control it, as weve now been told to stop using ours, and they think it might be feeding the growth as opposed to stopping it!

Next stop is surgical intervention..another operation for her...

dh used steroid cream as well for his over granulation.

riven, its where the skin heals but cant heal correctly because of the tube, and needs to be treated as otherwise it can grow onto the tube and cause problems.

dh explaining it to me in more detail, but hard to explain. he says it can look like my skin tags. he used to get it a fair bit. and it can grow quickly.

Ah that's a shame Mulsey. It's very unusual to become allergic to dressings, but I have seen reactions to adhesives (did you have the adhesive Lyofoam?).

Have you tried some of the other foam dressings, such as Tielle? Or did she react to those as well?

I hope you/she can avoid surgical intervention

Ive lost count of all the different dressings..it wasnt the adhesive lyofoam either. With all the different things Ive used, the skin around the site is a different colour now, and just doesnt look right...

Ive now been given a honey dressing to try, fingers crossed it helps a little.

Is it Mesitran that you've been given?

I find Mesitran good for mucky wounds but can cause overgranulation. It's also very moist so might make the surrounding skin too wet.

Has she had her tube long? I find over time gastrostomy sites can become discoloured and start protruding, as the rubbing of the tube can stimulate cell growth. It can be really hard to shift.

We used Hydrocautizone cream for Tobys g tube site. His wasnt over granulating a lot but it helped to nip it in the bud. I expect this cream is the first one you were given.

How long has she had the g tube?

I used to use diluted eucalyptus oil. It seemed to work pretty well but the nurses weren't happy with me using it as "there is no clinical evidence" so we had to stop. I do know that it has helped others though.

its a while since my dd had over granulation but we were prescribed eye/ear drops,sorry cant remember the name but they did work wonders.

I love dd's tube. What a difference to know she's getting everything she needs through it, to know that she's getting a full dose of all her medicines. She has made startling progress in other areas since she hasn't had to put all her energy into eating and drinking. And she does still eat orally. Yes, getting used to it was weird but you can get used to anything. Now the biggest issue I have with it is that before the op her tummy button was an inny, it's an outy now (lap thingy went through it and a stitch slipped); that bothers me more than the piece of plastic hanging out!
