I have a 5 year old who is under speech and langauge and she has told me to fill out a dla form as on the waiting list for camhs, she suspects adhd and asd But I’m currently stuck on question 40 “please tell us about the child’s health condition or disabilities”
what do I put in that section if she hasn’t got any diagnosis? What things do I add?
her eating is terrible
no danger awareness
speech
behaviour isn’t good
emotional child either cry’s or gets angry at everything
dont dress herself
I have to encourage to brush teeth
clumsy - always hurting himself and others
Too friendly with strangers
Lacks concentration won’t do activities for longer then 2 mins
Doesn’t sit still or for long period of times
Repeats himself
touching everything even when not allowed or told no
distracted easiky
Impulsive thinking & doing
Plays on her own
wakes up constantly in night
Runs off
Tapping feet constanlty
loves physical rough play ect
list goes on ..
