Son has been referred for an assessment for autism & adhd, on question 40 when asked about health condition or disability, because he has not been diagnosed how would I word this on the form?

I put on my form adhd - referred to camhs autism - referred to camhs

In this situation, when I complete forms, I write, e.g. “suspected autism” and “awaiting (or undergoing for those already mid assessment) assessment referred to X on Y.”

Hi I’m trying to fill in my child’s dla form, on the questions where it asks how often and how many minutes how do I put this down as literally constantly to the point I don’t even know how many times 😫 it’s stressing me out, I’ve also wrote a letter explaining it is constant but how am I meant to fill it in on them questions?
