Making a complaint about a teacher

Has anybody made a. Implant about a teacher in a. SEN school. Unfortunately I'm in a situation where I absolutely have to do this but don't know where to start or even how to word it. Any help or advice greatly appreciated

Follow the school’s complaints policy. Set out the events that have led up to you needing to complain. Explain any contact you have already had with the school about the matter. Request what action you would like e.g. an email reply or a meeting. Try not to make it overly emotional.
