Is pacing (running) up and down or around the room whilst looking at feet or head turned to the side always a sign of autism or could it also be sensory related ?

It can be related to other things, including sensory needs.

It's definitely sensory - my 14 year old dd does it. She has ADHD and autism.

Ok - so not necessary autism but a sign of a child with or out without autism who has sensory processing difficulty ?

Yes, it can be sensory processing difficulties (with or without autism). There are other possibilities too, though.
