Conners Test/Form

My child was recently referred to the community paediatrician for adhd. The school contacted me today to pick up the Conners Test Form.

A few questions if anyone knows the answers at all please!

  1. Do I need to fill out all the pages including the graph? The pages are all stuck together.
  2. Anyone know how to interpret vaguely what the scorings mean?

I've only completed the first page so far but roughly worked out with the IN her total score is 13 and for HY her total score is 18...looking at the graph it would means she's at the top 90 as she is only age 6.

I've looked online and I can't make head nor tail of it.

Any support welcome!

Hi I wouldn’t get too hung up trying to score it. You should just have one form of questions not any graphs or scoring systems as this is for the professional to assess and score as part of the process. You can of course look online.

School will also have been asked to fill in a questionnaire though it has fewer questions than the parents form.
