So when applying for DLA, do they contact the school for information/evidence etc? Or do I just provide everything myself. This is all completely new to me so I’m at a loss. Any advice would be so appreciated!!

DWP sometimes contact the school &/or other professionals involved. However, they don’t always, so don’t rely on them contacting anyone.

There is a section for someone who knows your child to complete, school could do this. Otherwise you should send copies of any reports you have to support your application.

I think there is space in the form for you to put the school's details but I'm not sure if they make contact. You should include as much evidence of your child's needs as you can, diagnosis letters, assessments, reports, and/or an EHCP if you have one. As other poster's have said, there's space for someone who knows your child well to describe their needs on your behalf.
There are a few online guides available to help you fill out the DLA form. I found the Cerebra guide helpful when applying for my autistic DS.
