Permanent exclusion and EHCNA advice

My child is 7 in yr 2 in mainstream. They have SPD and anxiety and have had multiple fixed term legal exclusions for violence, not following instructions and damaging property. Following an incident recently they received a fixed term exclusion again. HT has emailed me with a report of the incident, containing photos and eyewitness accounts. Their report does not include what lead to the incident - which by my child’s account goes against all OT recommendations as to how school should support them, and it also includes at least one factual inaccuracy. I know because I witnessed what is being inaccurately reported. We are due for a reintegration meeting and I suspect that the head will permanently exclude - can he at a reintegration meeting? Is it likely that the report has been prepared by the HT to present to govs in seeking consent to exclude? Should I raise the inaccuracy? And what about the fact that it doesn’t regard the circumstances leading to the incident? We are waiting for further OT and EP assessments as part of an EHCNA.
