Help with a DLA form

It's taken a lot for me to request a form. I'm surrounded by people who are judging parents that claim. FWIW I work full time and don't judge anyone who claims what their family is entitled to. DS awaiting his ASD diagnosis, he's 3, I've posted about this scenario before but we're now further forward. Anyway a health visitor check took place this morning, she asked if I was claiming and has convinced me.
I've called for the form this morning and I know I will struggle to fill it out. I know I minimise a lot of the extra/different ways to care for him. Is there anywhere that helps complete the forms? Health visitor has been useless, I'm afraid, I wouldn't ask them.

Thank you, do I also need a supporting statement/letter included? Could possibly ask nursery for that.

The form has a section that can be filled in by hv or nursery, assuming they are supportive of the claim - i.e. not denying any issues.

Send copies of any formal letters or reports as support. There is a section for you to list these.

Thank you - yes health visitor and nursery are aware and nursery are very supportive so will ask them

Cerebra guide is great. There are often local charities that will help. I’m in West Sussex and the reaching families charity talked us through it and will also check the forms and advise any changes before I send it. They’re soul destroying to fill in, I’ve really struggled but hoping they award it for DS and he can then access more help as well

Thanks everyone. I've since received the form and I'm finding it very draining. Currently writing a diary so when I come to write it I have something to work with. Got a meeting with nursery so asking them for a supportive statement. Thankfully they give plenty of time to fill it in
