What questions did they ask you for adhd referral?

Hi,

The referral is not just for ADHD and is for the neuro department, so could be for a number of conditions.

It's a very long and in-depth form which asks questions about your pregnancy, their birth, development, behaviour in multiple places e.g home & school, sensory issues and if any family members have been diagnosed with any neuro conditions.
