it has just turned up on my doorstep, sent by the physio. i have to fill it in.

The answers to some of the questions are
Independant
Supervision
mINIMAL
mODERATE
mAXIMUM (and its these last three I am having problems defining)

do i treat it like a DLA form and do worst case scenario? really lost with this. i presume portage used to fill it our before

Hi Fio - sorry, never heard of this let alone had to fill one in. Surely you shouldn't be expected to be able to fill it in by yourself? Can the physio guide you?

Who is it for? I guess if it is connected to getting services for your dd then I would take the DLA approach.
