Am applying for DLA for my almost 4 year old. There's a section in the form that CAN be filled out by someone who knows him like a teacher etc. I know his nursery are really busy so my question is has anyone sent this off without that section filled in? Think it's on page 8

We have the Paedtrician report which is being included

I didn't get that form filled in, as DS got awarded middlerate care & low rate mobility, but I already had quite a bit of evidence, including minuts from school meetings and DS's school SEN plan, which had everything the school would have been able to say anyway.

I would get it filled in. I think it does really help. It can be anyone who knows the child and has spent time with them. A grandparent? Godparent? Family friend? I have used a therapist before as well. If you feel nursery are the right people to do it but worry they are too busy to get around to it then I’d type up a statement stating his additional needs and the extra support he gets at nursery and ask them to sign and date it one day at drop off…

Spoke to the nursery this afternoon and they are gonna do it for me so was worth asking after all
