EHCP question

The nursery my DD 2.5 goes to are completing her EHCP plan for me. I've just tried to rang and ask but the manager is on holiday for the rest of this week.

She gave my husband this form for me to complete. Its probably a silly question but I'm a little lost with it? Do I just fill all these bits in as best as I can? She's non verbal some of the questions are hard to answer?

Answer as best you can, the process is so complicated that one form from you won't affect the outcome of it.

That form seems to be about what she likes and is comfortable with, likely so the professionals have an idea of how to interact with her so bear that in mind when filling it in.
