Need help with applying for assessment for dd CP/ASD

i know im an old hand having done DS,s but im really struggling.
on the letter that IPSEA recommend you write when requesting a statutory asessment it says "my reasons for beleiving that school cannot on its own provide asisstance-or something along those lines is:

DD has hemiplegia and suspected ASD, she,s incontinent and has speech and language difficulties.

the nursery have enough staff but i want her statement ready for when she starts reception.

she has very strict routines and this morning her usual lady wasnt there to give her breakfast so she refused to go in the room (she,s been going for 2 years and still like this) so i had to find the SENCO who is her afternoon lady and she took her.
this is happening frequently and the SENCO had to keep her all day a couple of weeks ago with her.

basically im just struggling to put my reasons down.

The school we would like our daughter to go to whilst, a very good school is set up towards the mainstream child who has little or no physical or emotional problems. With that in mind, we are requesting a statement for our daughter, who is wholly dependant on adults for her physical care, she is incontinent and had speach and language difficulties as well as hemiplegia and ASD. As you can imagine if she is not awarded a statement the school will be stretched so thin as they will be helping dd with her toileting and speach problems as well as her asd then the whole class will dip out as well as dd.

Sorry its been a while since I did a statement letter. but maybe it would be good to put that all the kids will loose out not just dd.

I've no experience of statements but will be watching this with interest & pinching your good ideas.
ds2 is in reception he has CP & ASD & no statement but have been discussing it with the psych.

If I coul recommend just one thing its do it your self, but I expect you know this already I should think. Have you thought of asking Partnership with Parents for help as well.
