Mistake you on dla form

I've made a mistake on dla form I ticked the wrong boxes on a page do I get a new form or just correct it?

Just correct it.

To make it look a bit neater, you can photocopy the page, tipp-ex out the wrong thing, photocopy it again and glue it over the top of the wrongly filled-in bit, then tick the correct boxes.

Also make a note at the back to say what you've done and why and which are the correctly ticked boxes so there's absolutely no doubt.

I would just print the whole form off again but the change of circumstances form isn't available online only the full form
