Herts cater card/passport

Hello - has anyone applied for this? I am not sure what the criteria is and couldn't find online. My DS is 9 with ASD, ADHD and hyper mobility. He has DLA at lower mobility rate and middle care.
I saw a post on Facebook about it but when I looked back couldn't see anything.
