Requesting an EHCP assessment

In the requesing letter how much detail do you go into with regards medical issues that are the cause the issues needing extra help at college?

I used the IPSEA model letter and also copied and sent in all recent medical letters.

I listed them and labelled them clearly, as I felt the letters from professionals will hold more weight than my opinion.

In my county there is a form to fill in to apply that I sent with the model letter and a lot of appendix. Work samples showing little to no progress, IEP's, health letters, attendance report from school.

I am new to it all but read lots around and came to the conclusion that letter/application form is what they make the first decision on so sent as much as possible.

Thank you dogdaysandcatnights. I’ve used the template from ipsea and it’s gone off to college for them to look over it and advise further.
