Parental statement for echp application

My dd (9) has severe health problems. She hasn’t been at school for 6weeks and we’ve been told she can’t go back until there are systems in place to assist her to remain safe in school.
She’s always had this condition but sadly she’s deteriorating. The ehcp has been turned down 4 times!! We are in process of gaining as much evidence as we can and giving the application another shot, this time going to appeal/MP etc if it’s not successful.

Can someone please help me in knowing how to write the parental part of the application. I have absolutely no idea how to start it. Is it a history of what she’s like in general or is it specific to how she can’t go to school due to her needs?
The latter would be very short and succinct as it’s black and white. No help, no go!!

Please point me in the right direction. I feel lost in my new normal.

You say the EHCP application has been turned down 4 times. Did you ever appeal the decision. I ask this because a lot of LAs routinely turn down the initial application and then cave in when you appeal.

My parental statement described my child from babyhood , problems in early childhood, difficulties in school, how he compared to his peers etc. So I started mine:
X was born on xx.xx.xx the youngst of three children. I then described what he was like as a baby and when we first noticed difficulties, then basically charted everything from toddler years to nursery school to primary school to secondary school etc, making sure to include any difficulties and referrals to outside agencies, eg SLT, OT. I also documented the progress he made through school and how this compared to his peers.

In my experience, for any form filling whether it be for EHCP, benefits or social services, you have to describe everything as it really is on the worst day.
